We are seeking a Part time, 0.6 FTE, Ongoing Community Health Nurse who is ready to contribute to positive outcomes for the people and communities we serve.

Role Description

The Community Health Nurse delivers culturally responsive care, including assessment, treatment, care planning, and health education.

The Community Health Nurse will be working with people experiencing homelessness and complex needs as part of a multidisciplinary team, and this includes outreach, clinics, and collaborative, strengths-based care with access to GPs and support services.

What you bring to the role

  • Registered Division 1 Nurse
  • Current practicing licence with AHPRA
  • Minimum 3 years postgraduate experience
  • Strong belief and personal alignment in cohealth Values and Mission
